What is Ayushman Bharat Yojana?
Ayushman Bharat Yojana, officially called Pradhan Mantri Jan Arogya Yojana (PM-JAY), is a health insurance scheme run by the Government of India. It was launched on 23 September 2018 by Prime Minister Narendra Modi in Ranchi, Jharkhand.
The scheme is managed by the National Health Authority (NHA), which works under the Ministry of Health and Family Welfare, and it runs jointly with state governments.

Eligibility under PM-JAY is largely based on the Socio-Economic Caste Census (SECC) 2011 data, along with certain automatic inclusion categories.

- Families with no adult member between 16–59 years

- Families living in kutcha (temporary) housing
- Landless households earning through manual labour

How to Apply for Ayushman Bharat Yojana
In west bengal the ruling government instructed ASHA workers to distribute ayushman bharat forms to the eligible households(PHH, SPHH,AAY ration card holders).Families need to fill this form and submit the same with the ASHA workers. PHH, SPHH,AAY ration card holders can avail the Ayushman Bharat card easily from this process.
After that the data will be submitted to the relevant database,

- Visit the official beneficiary portal at beneficiary.nha.gov.in, or download the Ayushman App from the Play Store.
- Click on “Login as Beneficiary” and enter your registered mobile number along with the OTP received.

- Complete the captcha verification and log in to the dashboard.
- Select your state, scheme (PM-JAY), and district.
- Search for your name using your Aadhaar number, Ration Card number, or family details.
- If your name appears as eligible, click on “e-KYC” to verify your identity through Aadhaar OTP, fingerprint, or facial authentication.
- Upload the required documents and your photograph.

Offline Application Process
- Visit your nearest Common Service Centre (CSC) carry your Aadhaar card and other documents. A small facilitation fee may apply at CSCs.
- Alternatively, visit an empanelled hospital, where an Ayushman Mitra (a helpdesk staff member) will assist you with registration and e-KYC on the spot.
- You can also approach your local Anganwadi worker, ASHA worker, or Gram Panchayat office, especially in rural areas, for help with enrolment.
- Once your identity and eligibility are verified, your Ayushman Card will be generated, either printed on the spot or made available for download.
How to Download the Ayushman Bharat Card
- Go to beneficiary.nha.gov.in and log in with your mobile number and OTP.
- Select your scheme, state, and district, then search your name.
- If your e-KYC is already complete, you will see a “Download Card” option.
- Click it, and your Ayushman Card will be downloaded as a PDF, ready to print or save on your phone.
- If e-KYC is not complete yet, finish that step first the download option appears only after successful verification.
How to Check Application Status
- Visit beneficiary.nha.gov.in or open the Ayushman App.
- Log in using your registered mobile number and OTP.
- Go to the “Application Status” or “My Applications” section.
